Cancellation Policy – Registrations

Cancellations postmarked by December 31, 2008, are entitled to a cash refund. Cancellations must be sent in writing, along with processed name badge(s) to: MDDS, 3690 S. Yosemite St., #200, Denver, CO 80237.  A $15.00 administrative fee will automatically be deducted from the refund for each cancelled registration. No refunds of any kind will be given for cancellations postmarked after December 31, 2008.

Cancellation Policy – Hands-on Courses/Ticketed Educational Events

Cancellations postmarked by December 31, 2008, are entitled to a cash refund. A $15 administrative fee automatically will be deducted from the refund of each cancelled course/event. No refunds of any kind will be given for cancellations postmarked after December 31, 2008.

 

Confirmation of Registration

A confirmation is sent for each registration received. If you register and do not receive a confirmation, please call RMDC 2009 Registration at (303) 488-9700 to verify that your registration was received. If you pre-register and do not receive your name badge and event tickets (if applicable) by December 31, 2008 please call RMDC 2009 Registration at (303) 488-9700 to verify that your registration was received.

Deadlines, Pre-Registration

The special early registration deadline is November 14, 2008. The pre-registration deadline is December 19, 2008. Registration forms must be postmarked by these deadlines to receive the registration fee that corresponds to each deadline.

Please remember that the busy holiday mailing season causes mailing delays on both ends of the registration process. Please register early to avoid delays.

Last Day for Receipt of Registrations
No registrations will be accepted by fax or mail after January 5, 2009. Those people needing to register after January 5, 2009 must register online at www.rmdconline.com or on site at the Colorado Convention Center beginning Thursday, January 15, 2009 at 7:15 a.m.

Click here to access secure on-line registration. On-line registrations will be accepted from early-August 2008 through January 17, 2009 at 10:00 a.m. Dentists who register online receive a $10 discount.

On-site Registration
On-site registrations will be processed all three days of the convention in Lobby “C” of the Colorado Convention Center. Cash, checks, VISA, Master Card, American Express, Discover and Novus are accepted.

Registration Audits
MDDS reserves the right to audit, at any time, any registration category for accuracy.

Substitution Policy
Substitutions are allowed and will be processed free-of-charge. To register a substitute, please send the following to RMDC 2009 Registration*: 1) the name badge of the person who will not attend the convention; 2) the name and registration category of the substitute; 3) Payment for the difference in registration fee if substitute’s category and fee are different than original registrant’s category and fee.

Substitutions will be processed through January 5, 2009. After January 5, 2009, all substitutions will be processed on site at the Special Assistance Counter in Lobby “C” at the Colorado Convention Center.

Registration Name Badges

Standard Registration

The standard attendee name badge allows admittance to all lectures* at the Colorado Convention Center and the Hyatt Regency Denver at Colorado Convention Center, the Opening Session on Thursday, January 15, the Expo Hall featuring over 325 booths and the TGIF Party on Friday, January 16. Your name badge must be visible as you enter each area of the convention. 

 

*Not included in standard registration fees are: all food functions, hands-on courses and ticketed special events.

 

Expo Hall Only Registration

Exhibitors and family members who register as “Expo Hall Only” will receive name badges that allow admittance to only the Expo Hall, Opening Session and TGIF Party. These name badges must be visible as you enter the Expo Hall, the Opening Session and the TGIF Party.

 

Saturday Expo Hall Only Registration

If you register for a Saturday Expo Hall Only pass, you will be admitted to the Expo Hall only on Saturday, January 17, 2009, from 10:00 a.m. to 1:45 p.m. You will not be admitted to the Expo Hall any other day.

Receiving Name Badges
Attendees can receive their name badges via First Class Mail or Fed Ex 2-Day Delivery (an extra charge applies).

Name badges will not be mailed until December 1, 2008. After December 1, 2008, please allow at least two weeks for processing. Please be sure to check your name badges for accuracy.

Registrations received after December 19, 2008 cannot be guaranteed to arrive back at your office if mailed by first class mail. If you send your registration form late, you may want to consider having your name badges sent via Fed Ex.

Misspelled Name Badges
If you pre-register and your name badge is misspelled, please return it to RMDC 2009 Registration, PO Box 571, Brookfield, IL 60513-0571 so a replacement badge may be issued. If time allows, your corrected name badge will be returned to you prior to the convention. If your corrected name badge cannot be mailed, it will be held for pick up on site at the registration counter in Lobby “C” at the Colorado Convention Center.

Replacement Name Badges

Should you misplace or forget your name badge, a replacement badge can be issued to you at the Special Assistance counter in Lobby “C” of the Colorado Convention Center. To obtain a replacement badge, you must present a photo ID. Replacement badges will be issued free-of-charge.

Special Registration Categories

Active Duty Military Dentists
Active duty military dentists must provide a copy of their active duty status identification with their registration form.

Foreign Dental Association Members
Dentists who are members of a foreign dental association must provide a copy of membership status with the registration form.

New Dentists
Dentists registering under the “New Dentist” category must be ADA members who have been in practice for 1 to 3 years. Specialists who were in general practice for more than 3 years after graduating from dental school do not qualify for this registration category.

General Practice Residents (GPR)
General practice residents must provide a copy of hospital affiliation of GPR status to be granted this registration category. GPRs registering after December 19, 2008 are required to pay a $89 registration fee.

Students – attending an ADA accredited school
Dental hygiene, dental assisting, laboratory technician, and dental students must show evidence of current attendance at an ADA accredited institution in their respective fields of study to gain admittance under this special category (e.g., student ID). In addition, students registering in this category may not be earning a wage in the dental profession (e.g., dental office, laboratory, etc.). Students registering after December 19, 2008 are required to pay a $89 registration fee.

Students – attending a non-ADA accredited school
Dental hygiene, dental assisting, laboratory technician, and dental students attending a non-accredited institution must pay a registration fee of $35 and must show evidence of current attendance in their respective fields of study to gain admittance under this category (e.g., student ID). In addition, students registering in this category may not be earning a wage in the dental profession (e.g., dental office, laboratory, etc.). Students registering after December 19, 2008 are required to pay a $89 registration fee.

Family Members
Family members who plan to visit only the Expo Hall and/or the TGIF Party may register at no charge. Family members who plan to attend educational seminars must pay the appropriate registration fee.

Guest
If a person does not qualify for registration under one of the other categories listed, he or she must register as a guest and pay the appropriate registration fee.
