Welcome to the Metro Denver Dental Foundation’s Smile Again Program. Thank you for your assistance in making this program a success for your clients and our volunteer dentists. Together we will make a difference.
- REVISED! Patient Referral Request Form – use this form to request services for qualifying patients
- Program Guidelines – includes information on patient eligibility and a recommended referral process to ensure patient compliance in the program
- Rules and Expectations – Make certain that your referrals understand and agree to the conditions of program enrollment.
- SAMPLE Instructions to Patient Form – WHEN A PATIENT HAS BEEN PLACED WITH A DENTIST, YOU WILL BE SENT “INSTRUCTIONS TO PATIENT." SIMPLY PLACE IT IN THE PATIENT FOLDER TO COMPLETE THE ORIENTATION PACKET.
