Welcome to the Smile Again Program. Congratulations on your acceptance in our program.
You and your dentist are partners in maintaining your oral health. Take time to ask questions about how this program works and follow your dentist's advice. When beginning your dental treatment, there are a few things you should know about what to expect both during the first visit and over the course of treatment.
You will ultimately ensure the success or failure of the treatment process. Trust your dentist and the staff. Because we want you to be successful, please be aware that if you do not show up for a scheduled appointment, without following your dental office’s policies, you will be automatically dismissed from the program.You will receive the best care possible; care that you so deserve. Your Dental Visit: What to Expect – a resource guide that includes information about the program rules, your first dental visit, who you will meet in the dental office and dental terms
Waiver and Release Form – you must give a signed copy of this form to your dental office at your first appointment and you must fax a signed copy to (303) 488-0177
Satisfaction Interview – we want your feedback on your experience in the program, so please complete and return this form at the end of your treatment
